Community First Services and Supports (CFSS)

Community First Services and Supports (CFSS) is a program in Minnesota that offers assistance to individuals with disabilities, helping them live independently in their communities. It allows participants to choose and direct their own care services. CFSS was designed to replace the Personal Care Assistance (PCA) program and officially launched on October 1, 2024.

Eligibility depends on the number of dependencies a person has in activities of daily living (such as bathing, dressing, eating, and mobility).

Eligibility is tied to Medical Assistance programs, meaning people enrolled in these services may qualify for CFSS. 

CFSS Models

Budget Model

 

  • Control over Hiring: Participants serve as the employer and have the ability to hire, train, and supervise their support workers directly. 


  • Flexible Budget: Participants receive a set budget to manage their services, including purchasing goods or hiring workers based on their needs.


  • Financial Management Support: A Financial Management Services (FMS) provider assists with payroll and managing the budget.


  • Personalized Service Delivery Plan: Participants create a customized plan that aligns with their unique care requirements and goals.

Agency Model

 

  • Agency as Employer: A CFSS provider agency is responsible for hiring, training, supervising, and paying support workers.


  • Collaborative Plan Management: The agency and participant work together to monitor and adjust the service plan as needed.


  • No Employer Responsibilities: Participants do not manage payroll or taxes; the agency handles these tasks.


  • Optional FMS Provider: If purchasing goods or services, the participant must choose a Financial Management Services (FMS) provider.
