Community First Services and Supports (CFSS) is a program in Minnesota that offers assistance to individuals with disabilities, helping them live independently in their communities. It allows participants to choose and direct their own care services. CFSS was designed to replace the Personal Care Assistance (PCA) program and officially launched on October 1, 2024.
Older adults who require help with daily living activities or health-related tasks to remain in their homes and communities.
Individuals with physical, cognitive, or developmental disabilities who need assistance with daily activities and support in their homes or communities.
Eligibility depends on the number of dependencies a person has in activities of daily living (such as bathing, dressing, eating, and mobility).
Eligibility is tied to Medical Assistance programs, meaning people enrolled in these services may qualify for CFSS.
